Welcome to Cardiovascular Consultants, P.A.! We are here to provide you with quality cardiac care and service. Prior to your first visit, you will be asked to complete the registration form below. We will be glad to answer any questions you may have during your visit.
We are excited to offer CVC patients the web-based patient portal. This tool enables patients to view visit summaries, past and future appointment dates and times, submit prescription refill requests and update personal information all through a secure connection. If your patient portal account has not been activated, call a CVC associate at one of our offices: Takoma Park 301-891-7000 or Rockville 301-990-0040. You may also send an email to info@cvcheartcare.com.
NOTE: Upon activation, you will receive an email from reminders @ eclinicalmail.com with the subject of “Patient portal access information.”
Scheduled for testing?
Please complete the following form if you are scheduled for cardiac testing such as: stress testing, echocardiography, vascular testing, or ambulatory arrhythmia monitoring (Holter monitor).
TESTING INSTRUCTIONS
If you are scheduled to have a stress test performed in our office, you will need to prepare yourself prior to the testing day. Failure to carefully follow instructions may result in rescheduling and any applicable charges. Therefore, we ask that you read the instructions thoroughly. Please let us know if you have questions about the information contained in the instructions or if you are not sure which instructions you should read.

NOTICE OF PRIVACY PRACTICES
We are required by law to provide you with our “Notice of Privacy Practices.” Once you have read the privacy practices, we will ask that you provide us with a signed “Acknowledgement of Receipt of Privacy Practices” form. If you have questions, bring the forms with you and someone will be glad to assist you when you arrive.

NEW PATIENT CHECK LIST:
- Please let us know if there are records or recent tests we need to obtain to make your consultation complete. If you have copies of these tests, please bring them with you. This includes test results from a hospital admission, an emergency room visit, or recent tests from another physician.
- Please bring an accurate list of all current medications, non-prescription drugs, and herbal supplements for review. It is even better if you can bring the prescription containers.
- Please bring all of your insurance information and a form of identification.
- Please bring a referral from your primary physician if your insurance requires you to do so.
- Please bring your completed New Patient Consultation Form and a signed Acknowledgement of Receipt of Privacy Notice form.
If you cannot make your appointment, please be sure to call us 24-hours in advance. We do have a missed appointment fee of $25.
